
Submit a Survey Build Request
Your LBA management team wants so support your need for information.
If you are the leader of any committee, sub-committee, or if you manage a group, you can request we put together a poll or survey for you. Here’s how:
- Touch base with any Board member (find their names and contact information on TownSquare.) Describe the form you need and ask them for verbal permission.
- Once you have that permission, fill out the form below.
- Once we receive your request, we’ll build your survey or poll, and will send you a link for review.
- You’ll review the form we provide and send us corrections / additions. We’ll ask for approval before we supply the final link to the survey/poll so you can send it out to those who should fill it out for you.

Here are some survey / poll best practices to help you:
- Ask a limited number of questions – no more than 5-6 (in addition to name and contact info). People are often willing to complete a short poll or survey but not a long one.
- Multiple choice answers are always preferable to fill-in-the-blank questions. Again – people will balk at having to think up creative answers. If you aren’t sure what choices to provide, you can always include “other” – and we’ll give them a place to write out their own.
- Not all questions need to require an answer. If it’s OK for someone to submit anonymously, then don’t require the answer. If it’s OK for someone to skip a question, then don’t require an answer. (Required answers get a red star and the survey taker won’t be able to submit without it being completed – see above for examples.)
